Creating Surveys

 
 

To create surveys for your location, groups, and subjects, follow the steps outlined below. Location admin with the correct permissions are able to create surveys. These then become available to groups for the purpose of rounding on subjects.

To create a new survey:

  1. Select “Survey builder” from the side navigation

  2. Click “+Survey” at the top left of the screen

3. Add the title / survey name

4. Select which group or groups you want your survey to be available for (note you can leave this blank if you haven’t created the correct groups yet)

5. The first question of the survey is blank and set to the question type “Multiple choice scale: 2 responses”. The answers will be assigned a value unless you choose to change the value. The answers for scale questions will automatically apply a scale of 1-10. Example: a scale question with two responses would weight the first response a 1 and the second response a 10. However, all response weights can be edited to any whole number between 1 and 10 (1 being the lowest score and 10 being the highest score)

6. Note that multiple choice is the only question type that can have scores. (Checkbox and free-form cannot.) For responses that are neither positive or negative you can remove the score (leave it blank) and the response will not have a score associated with it

  • The question type response options are as follows:

    • Multiple choice scale:

      1. 2 answer options

      2. 3 answer options

      3. 4 answer options

      4. 5 answer options

      5. 10 answer options

      6. Custom

    • Checkbox

    • Custom – allows you to pick the number of responses and assign a score between 1-10

    • Free form – allows the you to create a question that has a free form response text field

    8. Choose your category (optional) – this assigns your question to a category at the location

Adding meta tags to your responses

This allows you to create a followup tags based on how your subject responds. This powerful feature can be used to break down a survey response and perform more thorough service recovery. For example, you might ask a patient how the food is. They respond it’s not good. When this happens, the rounder can be prompted with a series of tags to select from such as “too cold” or “too bland”

Adding alerts, and users that will be alerted to surveys:

  1. To add an alert to a survey, click the alert checkbox to the right of the question response. This will generate an alert that will appear on the dashboard, or under a user’s alerts (see next bullet point)

  2. To directly alert users at a location, you will need to choose them from the user alert pool. There are two types of user alert pools:

    • Users at a location – This is a drop down list containing all users at the location. You can select up to 5 users that will be alerted based on the question’s response

    • Users with the correct category – This is a drop down list containing all users at the location that have been assigned category you chose for your question. You can select up to 5 users that will be alerted based on the question’s response.

Adding additional questions and saving your survey:

  1. To add more questions click "+ questions below”. You are also able to insert questions between existing questions and swap questions with one another

  2. Click “Create” in the lower right corner of the form to save your survey. If you want to leave your survey in draft form, uncheck the box next to “Save as active”. This will set your survey to an inactive state
